There have been a few games where the only difference between "easy" and "hard" mode is that easy is just more enjoyable and requires me to grind a lot less.
There was some action RPG where the only difference between easy and hard was that you leveled faster and you had more options open to you in a skill tree on easy mode. Hard mode was literally just a slog through enemies with more health and less rewards.
If Hard mode actually feels like it will be more enjoyable, I'll gladly do it on my first run.

It goes like this if the game only has easy, normal, or hard then I choose hard, unless the game is known for being challenging in normal to begin with (like underrail). If it has more than three difficulties I choose the second to last hardest (heroic on halo)

This, you can never be sure that the difference between normal and hard isn't every enemy being a bulletsponge. It also helps you avoid bullshit like Metro LL's retarded Ranger Hardcore mode that removes hud elements like QTE prompts and navigating your inventory while Ranger Easy plays exactly the same but just minimalizes the hud instead of removing vital information.
I always pick medium. The reason being that it is usually how the game was made, but then some of their testers felt it was too easy or too hard and thus make those modes to cater to them.

It's not enjoyable difficulty though. Good difficulty changes will make the AI smarter and introduce more/different enemies in order to make the player strategise more instead of just shooting more.

Fight is longer, enemies hit you more, you die.
It requires you to play good more consistently.
It can be tedious in single enemy fights but it definitely adds to the difficulty in mob fights. since you can't turn it into a 1v1 as easily.

Hardest difficulty is the only way to go. That's how the devs want you to play the game. They just think "oh shit, this maybe is too hard to all those casual plebs" and make medium. And then they think "hey, maybe this is still too hard for fucking retards" and make easy.
I research what the difficulty settings change and then pick. Sick of going very hard and then enemies take bullets to the head like they're made of steel. Makes the game slow and boring as fuck.
Hard difficulty should mean you die in 2 hits and so do your enemies.

I remember playing about the first 1/4 of the game on nightmare forgetting that I had even set it to that difficulty. Game starts getting harder and die more often. Get to one part I just can't get past.
