What games have the most realistic gore?

I'm not talking about sick shit like torturing people, but realistic body effects like in warfare etc... losing limbs and other parts of the body for example. Or blood which splatters realistically.

It is very common in anime, but far less common in games. I want basically Hellsing: The game.
