If you're referring the the Mojave, that's because the NCR are relatively new there and haven't properly established themselves yet. That's a big part of the story, they're want to take control of the dam and take New Vegas for the NCR. Until then, the Mojave isn't NCR territory.

That's more subjective though so just look through the highest rated ones. part from that, it's been too long since I played, I don't remember my own setup. I do recommend keeping your mod load light though, you really don't want to go too over the top or there's going to be some serious instability.

For atmosphere I use Interior Lighting Overhaul, Realistic Wasteland Lighting, Electro City, and the Flashlight mod. Electro City unfortunately tries to do more than it is supposed and messes with the lore a little bit. Other than that, it's solid. Project Nevada is pretty good. It's pretty customizable and I only use a couple of the features but it has a lot to offer. Others of note are Jswayer, More Perks, Elainor's Sink Mod.

In Fallout there are governments, they just aren't very large because of the lack of resources available to them, and the degenerated state of the area which means transportation and communication are difficult, plus a heap of hostile organizations. In 40k it's just a result of extremely dangerous threats along with technological stagnation, probably resulting from the monopoly that the Mechanicus (or Mechanicum, whatever it's called) has on anything vaguely mechanical. Also it's supposed to be 'grimdark', that's the point. Hell,the phrase came from the series' tag line.

This like asking why horror movies are always trying to scare you or comedies trying to make you laugh. The endless struggle and obstacles are fundamental to the genre.

And anyway in Fallout 1 there were multiple established successful towns - Junktown, LA boneyard, The Hub, Sandy Shades. Which are all doing fine once you taken into context that over 90% of all humans died, of that about 2% are mutants or basically retarded (bandits), there was basically a complete reset of human society (vast majority of previous records were lost, in 1 and 2 before the 50's shit meme that 3 started), and all of this in only 84 years.

While I'm not a big fan of Bethesda Fallout, it's made pretty clear that unlike the degenerate eastern states, California has grown to a point that it's come back to a mostly normal state by the time New Vegas happens, and all that's left is Manifest Destiny.

Honestly while I'd prefer New Orleans, having a Fallout game that takes place in advanced California would be neato.
