The "dear friend" bit is a seriously brilliant piece of literature. Without spelling it out, the author makes the reader empathize with both Geralt and Yennefer. Geralt has to beg for help from his ex-girlfriend, and he has no idea how to address her in the letter. Likewise, Yennefer is offended by Geralt's choice of word and uses it as an opportunity to rib him whilst acknowledging his groveling. You can just feel how each character feels in this moment.
