Two thumbsticks in a sensible position.

Mess around with the four buttons so that you can put them on the equivalent of a d-pad. Then make both the d-pad and the buttons match. You can then let players play with either as the controller and the other as the button inputs.

Have the controller capable of storing game save data, so when you plug it into the system, you can just play from your saves and your controller settings.

It's got a heartbeat sensor so it can tell you how hardcore your gaming sesh is. It can use it to fuck with you in horror games or be like a realistic heartbeat when you're in a verses game (Like you can hear if the last guy is frantically shitting himself)

Plus I guess it can tell if the player is holding the controller or not. So if it's put down the game can be like "Oh shit the player doesn't have the controller in their hands, I should/shouldn't be a wee shite and throw a QTE out"

Tetris 64 did that. It adjusted the speed based on your heart rate.

A grip-strength button on it's sides. Press the controller hard to make the game do things like grabbing something.
