Liquid really did possess Ocelot in 2. For some reason Ocelot was sensitive to ghosts (we find out why in 3) but Ocelot realized it happened at the most inopportune moments so he had the arm removed and got a robot on instead and just hypontized himself in 4.

That's why the camera zooms in on his robot arm in 4. To show the player that its obviously not Liquid's arm that was in 2.

I think MGSV actually felt kind of realistic. It was the one that hurt the most to kill soldiers because with the voices and animations, they did such a good job showing the pain of war. If a soldier stumbled upon a dead body, he is actually fucking panicked.

The reason liquid could do it was both a mix of it being his arm and that hes the ends son. In the book and the official comic, the end literally insults the shit out of his son for letting liquid take over like that and comments on how much stronger he should be because hes HIS son and HES alive.

Liquid, while seeing the advantages of being liquid, also sees how bad it is that he cant control it and it happens w/e liquid wants it to happen. So he has it replaced with a nanomachine arm and does the hypnotherapy.

TL;DNR
It was liquid because Ends son, but between 2 and 4 he replaces it with fake arm and hypno and he fakes it from then on
