I suppose you could argue the idea is something along the lines of deeper penetration required to reach vital organs/higher volume of blood being able to be lost before losing consciousness or bear mode guys actually being tough or whatever.

But really, health in most games is used in completely abstract sense that doesn't map to any real-world attribute so health values are arbitrarily assigned as well. Shouldn't all bears have roughly the same amount of "health" (in some concrete sense like amount of tissue damage they can sustain before being incapacitated) for example? No, according to many a game in which high-level bears may have tens of times higher health values. Higher health on fat enemies is just as abstract as higher health on high level enemies, it's just a visual cue for tough opponents and there's nothing more to it.

You realize that vidya "health" does not actually exist, and in no way represents actually being a healthy individual, or that the character will live to a long age without ailments? Game health is more a indetermined mixture of luck, courage, the ability to ignore pain and sheer adrenaline fueled frenzy.
And no, it should not be the other way around, since fat is a physical barrier that whatever weapon needs to pass before injuring vital organs or other critical parts.
Ask a surgeon whose heart is easier to get to, a lean healthy person´s or a fat man´s.
