Getting singled out and focused down is just what happens when you play healer in any team game like this. Anybody with a brain wants to shut down the healer ASAP.

Luckily all the healers in Overwatch can keep themselves alive against divers and assassins if they have decent positioning, either by running away or fighting back (zenyatta)

Primary the discord on important targets. If they have Pharah, put it on her until she's down.

If they have Winston jumping on you, discord him and have the team focus him before he gets away.

Give discord to tanks who get hooked or pinned and it will turn into a kill.

I usually use Zen's right click most of the time. Once you land a volley on an enemy, throw discord and finish them off with left clicks.
