Even ignoring the shrapnel, the pressure from an explosion can still kill you. And then add in the shrapnel, hundreds of small metal fragments flying at you at really high speed.

Either way, the explosion creates such pressure, that it alone can rip human skin off and totally throw you off balance. Even a whole floor off.

There are many different kinds of grenades. The most lethal one and what you usually think of is a shrapnel grenade like pic related. It has a metal shell with grooves on it and upon detonation, the shell will break around the grooves and send small cubes of metal in every direction. The explosive itself is usually very small, around 1 gram of a fast reacting explosive such as hexatol, which is usually transported separately from the shell and is screwed into the shell when entering a combat situation. The deadly radius is about 10 meter and severley injuring to about 30 meters.

is not true, they're still a part of arsenal of a soldier. They have a defensive purpose and are thrown usually from trenches and similar land structures and landscape features that can shield you from the explosion.

The amount of explosives you can put in one is pretty limited so pressure alone at best can hope to do some internal damage to lungs if you literally have one slammed into your stomach.

I remember XIII being sort of realistic with its grenades. Yeah, the motherfucking comic book game. I experimented once and tossed a grenade into a room. The walls were covered with several holes where shrapnel had punctured them.
I thought it was pretty cool. Not only does it cause an explosion, but it sends several projectiles in random directions. Probably far from realistic, but at least they put some effort in.
