I find that personal appearance has a lot more to do with how people percieve you than actual activities. If you go on a bus and play a handheld, but you're generally well dressed and groomed, people do not tend to care. If you're an overweight neckbeard and wearing a fedora, then the handheld further cements their perception of you.

Handheld gaming systems pretty much only have video games, or are at least predominantly used for them.

Smartphones are used for all sorts of things. Most people aren't going to inspect your screen, so for all they know, you're texting, reading, browsing... anything. Even if you're gaming, it's probably something like Candy Crash, which is a minigame you're likely not very attached to, so it's cool.
