What is the most realistic depiction of ancient to medieval warfare on film?
Not GoT-tier shit with both sides running at each other screaming but actual tactics and formations being used.

The Alexander movie with Colin Farrel had some great battles, all based on the written accounts we have of the battles Alexander fought.
LotR actually displays traditional medieval style tactics albeit on a very rudimentary level and minus all the fantasy shit. The Battle of Pelenor Fields shows flanking maneuvers, proper siege formations and tactics, and general troop placement/deployment
early modern warfare but Waterloo (1970) is great.

The battle scene in Spartacus was neat. Don't know how realistic it was though.
Battles in The Last Kingdom are pretty spot on
Monty Python and the Holy Grail.
