Internal consistency. Dragons and a woman giving birth to a cloud are demonstrated to exist in the GoT world and so we buy it whereas the apparent inability of fatties to lose weight is not addressed. But even aside from that it's a matter of comparison between real life and fiction. We're more willing to accept the more fantastical elements of fantasy worlds since, as with dragons and magic, we don't have any comparison in real life. But when it comes to Sam still being a fat fuck we know how the body stores and burns fat and so that's why we're more critical of the more down to earth aspects of fantasy.

On the other hand, they haven't shown anything to suggest weight loss works any different in westeros, so him not losing an ounce is jarring.
