How does a house come to power? I'd guess before the seven kingdoms became official there were large "tribes" using fear flayed man would have help scare away raiders. Over time the bolton tribe became a major player and were made a house when the kingdom was formed and they just kept the symbol. Think savages becoming civilized in time.

Something like that sounds plausible to me. With Bolton, I'd imagine that their tribe was already unhinged to begin with (similarly like those cannibals beyond the wall) and just continued to refine their ways over the ages. Perhaps they even dialed it down to keep up a mask of sanity towards the surrounding houses - e.g. up until the red wedding, Roose seemed like a reasonable ally of the Starks and the North.

The theory I heard is the Boltons used to be the rivals of house Stark for thousands of years. In the past the Starks were more proficient at warging, so the Boltons took on skinning as a kind of counter to it (the same way you'd skin an animal you killed, you'd skin the Stark you killed).

These elaborate or super terrible tortures/executions were reserved for especially terrible crimes or to make an example. A common soldier or peasant is never going to be subjected to that. Flaying alive and burning alive are just about the worst kinds of death a common soldier could experience. Flaying more so because the nerves aren't burnt like in burning so you feel excruciating pain until you die or go unconscious.
