In Scrubs, JD is actually not a patient at all. That all takes place in his head. In reality, he's a patient at a mental institution and built up this elaborate situation in his head where he's a doctor and has all these friends. The drugs are what cause the random quirky hallucinations in his head. Turk, Elliot, and Carla are just the orderlies that come in to the common patient area to give JD and the other patients their medication for the day. All the other recurring characters are other doctors at the institution, and the "patients" that JD has are actually specialists or other doctors visiting the institution that visit but then leave. Whether they "die" or get better in JD's version of things is irrelevant-- in reality they're just leaving the asylum.

I forgot to add that Dr. Cox IS a doctor in reality, he's just a mean doctor that comes in and is fairly aggressive with JD so JD sees him that way in his version of things.

He even made up Pam as an imaginary romance for his imaginary employee, then it spiraled out of control in Michael's imagination, making up further friends and adventures.
