The whole goddamn backstory is that the only reason he was a good guy was because of his wife. When the last link to her was destroyed(the dog) he lost that anchoring and slid back into being a goddamn monster that scares everyone because of the kind of shit he'll pull. They're not scared because he's after some low-level russian mobster, they're scared because now the genie's out of the bottle again. There's literally not a single time in the movie where he does a "good guy" thing, the only rules he follow are those of the assassin's guild thing, and that's out of necessity.

You don't become the most feared assassin in a world with an assassin's guild by being a good guy.
