They were separate, it was the American distributor that promoted it as a trilogy, this was not Leone's intention at first but he went along with it.

They're connected as in they're happening to the same character. but he's such a loner that his history doesn't really cohere into anything, so don't look for patterns, it's just three adventures of a bad motherfucker. More like how John Wayne's character in Hawks' last three Westerns is the same character but with different names - they don't cohere as a story but they're obviously three of a kind.
