Basically, there's nothing stopping him from doing exactly that, but his programming prevents him from doing so, so he can come off as more relatable to the crew, which is the entire point of an EMH to begin with. Otherwise, there's nothing stopping him from just being a disembodied voice and a bunch of medical instruments floating around the room, which would be off-putting to the flesh-and-blood crew, and therefore counterproductive to the purposes of a medic.
