Easy. It was the Organization's elegant, simple way of telling Tom Cruise's character that they could reach out and destroy both him and his family at any time, and that the only reason they hadn't already was that it wasn't worth the hassle, and that he would be wise to never EVER fuck with them again. I always found that scene amazing; so much meaning packed into such a simple act.

whats funny is the entire situation was clearly just for show. Implying the organization had no actual power beyond vague threats. For if they actually did something serious, they'd have infinitely more to lose than some new york doctor.
