The other capos were employees first. Sal, Paulie, Pussy and the gang were Tony's mates, so they got to hang with him. Hell, even Bobby did once Tony's sister in law sunk her claws into him.

Exactly. Tony always knew he was just a jumped up street rat with ideas above his abilities, that's why he had panic attacks.

He knew he was just a street thug like his old man and it preyed on his mind all the time.

The Sopranos was a good character study. Of a guy who pretty much got grandfathered into the business and only managed to hold onto it because his old friends were loyal to him.
