The restaurant was full except for that one table and it was made clear he was uncomfortable with the seating arrangement. His choices were back to the door or back to the bathroom, neither was good.

Come on. There was no reason to think he was gonna get shot, and there is still no reason to think that he was. The whole point is the anxiety you have now. He wasn't shot, but the price you pay for his lifestyle is that in every normal situation, there is the shadow of violence, a deep fear that's causing him to be depressed. It was a good ending.
