So in Salazar's tale, he specifically mentions that he first thought of Jack as looking like a little bird up in the crow's nest, but how did he know his name? Jack wasn't even a captain at that point, just another pirate. How does Salazar know his name?

Further - how does Jack, who as far as we know never again had dealings with Salazar even aware that he'd started thinking of him as "Jack the Sparrow"? How does anyone not trapped in the Devil's Triangle know?
