So how did Rohan pull out 6 billion horsemen not once but twice if they were a bunch of backwater towns with a total refugee population of merely a thousand?

It was only around 6 or 7 thousand people, on horseback that is pretty threatening. Rohan is fucking huge and spread out. It took weeks to rally that number. Reminder, Saruman was eating away at Rohan piece by piece so there was probably more that straight up died in the skirmishes with Isengard.
