The same mental issues and enablers that put them at 500+ pounds keep them from reversing course.

Any mental illness can be "cured" by just saying "do [x]" or "don't do [x]" but that's only a mechanical, here's-yer-problem-fella approach. Obviously an obese person can just "not eat a bunch of food", just as a person with OCD can just "not wash their hands 10 times in a row". But preventing that doesn't address the irrational fucked up brain pathways that led to them eating 2 large pizzas for a meal every day.

You should watch My 600 Pound Life some time to see just how fucked these people are.
