It's mostly the Indigo league that's the problem, they're sending ten year olds out alone in the woods with just their starters and expecting them to be just fine running around between cities, there's no formal training for wilderness survival, nothing about actually taking care of the pokemon you befriend, just battle techniques! We had upwards of 200 children die last year on the gym circuit, 200! It's absurd, and horrible, and it needs to change!
Yes, there's more problems than just that, but saving lives is my biggest issue with the League right now.

There hasn't been a war of any scale in nearly 20 years, dude, the last one kinda unified the world into regions and now we don't have the same problems. There's no reason to make Soldiers, and 95% of trainers don't even make it as far as Vi has, they just pack up after losing too many times to one gym.
(Feel free to ignore, I'm not sure of the canon for Lt. Surge's war but this is the best I could come up with)
