Engine isn't the problem. The problem is creating assets and content for a game like that, which takes hundreds of people. That's why we have hundreds of unfinished open-world survival games that are on Kickstarter and Steam Early Access and not a single proper open-world RPG has made it through that pipeline.

There are plenty of indie RPGs out there, but they're aware of their limitations. They don't set out with the goal of making Skyrim or The Witcher 3 for $100,000. They're almost invariably 2D and not open-world. Even Pillars of Eternity, which is one of the best received indie RPGs and had a relatively large budget (thanks to Kickstarter), kept things simple and restricted itself to a 2D game world.

Pretty sure Clovis did not have an hourglass figure before the transformation-- the waist got narrower and the hips got wider. After the change her cronies noticed that she "looked different" and this could likely be (partially) attributed to the fact that men and women walk differently. Clovis didn't have a particularly impressive height to begin with. Maybe he lost an inch of height but I don't think that particular change would really be enough to notice. As a girl she looks slight and petite, and the next frame we see her she's completely covered herself in baggy clothes.

Height probably wasn't particularly effected, although Clovis' body did go through a full transformation from male to female and some skeletal changes would have come with that.
