Depends on context.
Patient point of view, it can be as mild as just waking up, or they could have a conversation with the doc while only feeling little tugs due to local anesthesia, or painful to traumatic if anesthesia isn't available.
From a doc's point of view, it could be dealing with anything from annoyance from a dumb patient, to worrying about said patient loosing some functionality in the stitched area due to the injuries. And any other number of things.
To say nothing of things such as possible care quality, or the potential for self surgery, or something truly life threatening.

But short answer, I'd probably place physical descriptions secondary to the thoughts of those involved, as too much physical in medical stuff could come off edgy, and there's plenty of worry and mental nerve to work with. But still keep a good peppering of the physical, since it's important to the scene.
