That and the fact that he’s covered in bandages, plus currently bleeding from one of them

“Oh, my lovely angel, have you come here to heal my wounds~?” Tuco says as he greets you, lying on his bed and doing a small pose “Actually please pass me that spray on the counter” he says as he points to one

“Yup, that one, fast” Tuco says with a strained breath as you toss the can to him, before spraying it all over his wounds grunting for a while “Done…” He says taking a deep breath “Now~ for what do I owe you this visit?” Tuco says with his broken monitor barely manageing to make a “:)”
