I want to play in an ERE where the Athanatoi truly are without death, a horde of proud undead legionnaires who fight for the glory of Rome even beyond the pale.

I want to play in an ERE where the Muslims have Efreeti soldiers and flying carpets. I want to play in an ERE where the Germanic tribes have enlisted Knockers and all the other horrible dark things that live in the woods above them.

I made similar setting a few years ago. The Hagia Sophia was ground zero for magic returning, turns out it was built over a Cthuthonic portal (I wove in real life myths dating back to the founding of Byzantion). The Empire was wrecked (what with their capital suddenly turning into a necropolis of the damned), but everyone else was dealing with their own shit so invasions didn't happen.

Elves suddenly appear in the British Isles, and it turns out they're masters of manipulative bullshit.
Giants claw themselves out of Norwegian mountains, some go on rampages and othets set up their own neo-Norse kingdoms.
Orcs tumble out of portals and make a new Khanate, rampaging over the eastern steppes and causing the horse-lords to fight for their lives.
Pope suddenly has to decide if magic is godly and can be used to fight back or not. Catholic church splits as a result.
Etc etc.

It really isn't. It's clearly the ERE, the state haven't changed, only the culture have and culture is by its very definition fluid.
Anyone who argues otherwise can't be taken seriously as it would mean that old countries can't exist. Being a roman had long ago seized to be about ethnicity and had become about culture, manifested in obtaining roman citizenship which at the time of the ERE was given to all roman subjects.

Why not include the patriarch of Constantinople and to a lesser extent the other three in Antioch, Jerusalem and Alexandria?
Seems like it would make for obvious centers of new religious doctrine if the five (or four if the one in Constantinople died during the recurrence of Magic) patriarchs can't decide on a official united stance.

There actually was some of that. The Patriarch of Constantinople (in exile) focused on Healing magic, the other orthodox patriarchies had their own specialty. The Anti-pope was a generalist, all magic was allowed but was mediocre in most ways. The Bishop of Rome outlawed magic and specialised in negating it.
