Only if they show signs of weakness or treason. It's to keep your fighting force able to deal with any possible threat. It does tend to prevent the generals who have no sense of what's going on in a war.

Common misconception. You can serve loyally and be promoted by your superiors or gain rank by making a name for yourself. You can also rise by challenging your superior but only in honored combat with a lot of ceremony. Lots of strict rules for what doesn't and doesn't constitute a rightful challenge, honorable combat and victory. As the superior you are expected to face these challenges to your position. Merely ganking someone gets you blackballed and shunned from Klingon society and they still frown on murder. If you kill someone outside of honorable combat they are entitled to your blood.
