I've seen some art about centaurs that did work like that (double set of most organs, including the womb, so a fetus would start in the human half's womb and move to the horse half when large enough). I suppose that would have advantage of increased birth rates (since a centaur could then get double pregnant), although the logistical problems with such biology would really cause way too much trouble (not that centaurs make biological sense to begin with).
In this case I think the reason is simply visual (to make her look obviously pregnant) and/or Centorea not knowing much about how centaur sex/reproduction works.

Centaurs are technically a race of lamia, so while they aren't disposed to either good or evil, they have an inherent paranoia of gods and religion. This leads to the same problem you encounter with normal lamia, where they might go berserk at the sight of the party cleric and try to lynch them.

That being said, centaurs are often the lowest caste of lamia society, beneath even the often-mocked lamias whose lower halves look like turkeys, so they're much more likely to be sympathetic to humanoids, as long as those humanoids don't bring up their faith.

This will sound weird, even to me, but horse sweat isn't that bad. One of my fondest memories from when I was young was riding a horse in the mountains.
Camp was below the snowline, but being early spring there was plenty on the trail I was riding. Heading back down to camp, the smell of leather saddles, horse sweat, and the distance smell of wood smoke, all combining together like that, the rhythm of the horse breaking through the snow. After months of fighting they were cut from the frontlines and used for transportation of heavy machinery and other materials during the war. This was mostly in Europe though.

Eastern part of WWI (Ottoman Empire + Asia) had horses used in charges and raids across the desert and wild terrains.

If you were going to use horses for combat look towards the 1870s with either the Franco Prussian War, Prussian-Austro War, and American Civil War.

American Civil War would also be a pretty unimpressive setting for centaurs. The tactics used during that war were pretty much, "What if we took Napoleonic Warfare but took out all the artillery and heavy cavalry charges that make it work." It's a bit of an aside, but if ever there was a war that involved mass infantry being marched pointlessly to their death it was this one.

If I had to pick a real-world setting to include centaurs in it would be Great Game period Asian steppes (pic related). You've got built-in conflict with a wide variety of factions, solid themes of tradition versus technology and the importance of the family/tribe unit, and an overall very underutilized setting. It's exotic feeling without being too alien, and feel free to throw in a bumbling English researcher if people need a cultural touchstone to relate to. People can understand what's going on while at the same time almost certainly not knowing enough of the real history to constantly 'correct' you with irrelevant 'realism'.

Horses were very expensive investments in the middle ages, so much so that nobles often considered the lives of their horses to be more valuable than the lives of the lower classes below them. In addition, getting under a horse to stab it is a lot more difficult when the horse is fighting and there is someone on the horse trying to kill you. Crouching down that far puts you in an awkward stance and puts you at risk of getting trampled by either horses or other men.

I know I'm a bit late for the party, but:

After (prototype) rifles show up in 1860's, all Cavalry is doomed.
Once the only way to survive on the battlefield becomes either not to be shot at in the first place (and Cavalry is such a nice, big, visible target) or having so much armor you can shrug off hits (and not even horses can carry so much of it) the only way to pull off the Cavalry's trademark charge is either an ambush (and good luck trying to hide) or a shitty/cowardly enemy (who couldn't hit a broad side of a barn).

So, if you want a WW1 age setting, I'd really recommend the fringes: Asian steppes, Balkan Wars, Colonial Actions, Russian, Chinese or (early part of the) Spanish Civil War are your best bets here.

Cavalry as in "everybody line up and run at them" sure, but mounted rifles were a real thing for a pretty long time, including, even particularly durong the first world war.
And that's what centaurs would be best suited for anyway. They've got the battlefield mobility of cavalry but without 80% of the downsides that come from needing to ride unintelligent horses.
Treat them more like infantry that move faster rather than the tactical battering ram cavalry is and they'd do really well.
There's a strategic opening on a hill? Your centaur troops can get there before the enemy's humans, and the terrain advantage makes up for them being bigger targets.
