An interesting one to consider is familial connections, both literal and figurative. Thing goes through friends and family. Only way to stave it off is to be a hostile asshole, cutting yourself off. Town descends into riots as the people trying to care for evryone literally fucks it over.

With regards to the explanation of psychic, that's kind of how hysterical illness passes. Mass panic and groupthink... Although, now that you mention it, thinking certain thoughts would be an acceptible vector... granted, it results in a no-win scenario as every one develops various kind of anxiety complexes deperately trying not to think of whatever the vector thought is...

My idea: words said in anger.
The words or language doesn't matter, but if the infected speaks while angry, they infect all who hear it.
And the symptoms of the illness are particularly annoying and irritating, propagating the illness.

Since it's not all spoken words, it would be hard to identify at first, especially if it starts slowly.
Depending on the origin of the illness, another emotion, like sadness, lust, happiness, fear, etc could be more thematic.
Season to taste.

I also remember a great short about an explorer who came back from a trip to the deepest jungle to find the meaning of life.
He had succeeded.
It had driven him mad, obsessed with spreading the knowledge at all costs.
And anyone who heard it went entirely mad in their own way.
