How would you make a campaign where the big bad just wants to have friends?
Speaking with people drives them mad or kills them, but he wants to do it anyways for selfish reasons.

If you don't mind clichés. Make them the only one of something, trying to bring back or get back to that something except doing so requires a mini apocalypse.
Example.
Glory from Buffy. She wasn't really evil. Just a betrayed goddess from a different dimension cursed to live out a meaningless life on Earth. She only wanted to return to her world. Too bad that required ripping apart the fabric of reality.

This basically. Might makes right in most scenarios, and perhaps the big bad just wants some form of attention. Everyone is your friend if you're carrying a world ending artifact. Who wouldn't treat the big bad who just rolled into town like a king if they knew what he carried could tear the world asunder? It's sorta like the whole everything is free when you have a gun thing.
