Think of combat damage happening as a huge batch. If anything dies (grave yard) appropriate triggers then trigger

Okay so what happens is this

You declare attacks and play any fast effects, if not you pass priority to opponent who may play fast effects, if neither plays any it proceeds to blockers phase.

Opponent chooses blockers, you may play fast effects here or pass and opponent may do the same. If both pass proceed to combat damage step.

If any creatures with first/double strike they do their combat damage first here, end of this damage step both sides may play fast effects in active player/non-active player order.

second/regular combat damage step occurs here for regular creatures and creatures with double strike. Damage is dealt all at the same time, barring some exceptions aside if anyone is on 0 or less life at this point they lose the game as part of the game rules(state based actions) unless a card states otherwise(Platinum Angel, Angel's Grace) In a typical 2 player game any triggered effects that would be put on the stack here simply just do not happen because someone has lost.
