What sort of a test or a trial should a fallen paladin undergo in order to gain back his status?

Rejoin the work. Help put the world back together. Nothing shows a willingness to change like changing.
Smite a bigger badder evil.

I'd say it would depend on why he fell. Most likely, it's doing something that's more or less the opposite (providing that doing as such would be to the god's tenants, and not fucked by technicalities.)

This. The fallen paladin should have to demonstrate the virtue opposite the sin that caused his fall.

Fuck the evil out of a succubus.

One idea I think would be interesting is to test if he really is devoted or simply wants his power back and is willing to be "good" for their own benefit.
Put him in a situation in which to complete the quest he has to make some grave self sacrifice. Possibly put him in a situation in which he needs to give his life for the protection of the innocent. If he readily accepts the sacrifice then bring him back "reborn" with the power of the god as a shiny good paladin again proving both his faith and commitment to the good.

Have a cleric cast a single spell on them
sometimes, even one who seems to have fallen so hard, become so shrouded in darkness that they seem beyond redemption, has the spark of goodness within them.
a moment of mercy, of kindness can change a man in a moment, and they begin their long path back to the lightgo read les miserable to see how a person beaten so far into villiany, that he believes himself evil, goes back to the path of the righteous based on some kindness

Redemption through death.

What if that's the reason he fell in the first place?
