Beeple-beep beep! With their organization so faulty, especially the Departmento Munitorum, why hasn't the Imperial Guard shattered and decentralized? A decentralized set of smaller armies would be far more effective than what they currently have, and it makes sense for people to rebel and separate from an organization when it consistently fails them.

It's not. You completely misunderstood the point of those fluff bits.
As a whole, the Guard is highly effective. But because of scale (and traditional 40k over-the-top-ness,) the big-picture over small-picture thing militaries do has been pushed to an extreme.
Real-life armies in wartime are imprecise. They're trying to get the job done, even if that means a handfull of civilian casualties. They're trying to keep the army as a whole running; one private getting nothing but socks as his kit is a minor error.
The Departmento Munitorum are the same, but on a larger scale. They're trying to get the job done, even if that means a few planets of civilian casualties. They're trying to keep the whole Guard running; one division getting nothing but socks as equipment is a minor error on the scale they're working at.
They're focused on the big picture. Individual guardsmen and units are cogs in the machine. They get the impression that the system doesn't care about them, because it doesn't, it cares about keeping the Imperium standing. That, it does pretty well. Smaller armies might take better care of their soldiers, but they'd be worse at the job of defending humanity. Planetary Defense Forces are no replacement for the Guard.
