Pendragon is this, but the stuff is steeped pretty deeply in the Arthurian Legend stuff system wise but I'd still take a look, newer stuff in particular is pretty steller.

I'd grab GURPS for this, set it in TL 3, set in 1233 using stuff from Low Tech, Crusades and Martial Arts book.

I'd either have no magic for player characters or limit them to slow, careful spells, ritual path magic with no Adepts, battling the dark pagans and twisted, strange Orthodox heretics in the far north of the world.

Give the characters 200 points or so, with 50 points in flaws. Powerful, dramatic characters, but ones that aren't superhuman.

Teutonic Knights were not noblebright, they were Catholics happily slaughtering other Catholics or Orthodox Christians merely because their purpose was invalidated through conversions.

They're the perfect example of how holy knights could be anything but, their wars were fought to conquer land and form a kingdom, not convert the heathen.

also, you can push the game into some less explored historical environments like Polish-Lithuanian Commonwealth, Kievan Rus or medieval Russia, lands around the Black Sea etc. Too bad Microprose never got around making Darklands 2 because they planned to explore the historical setting mentioned and if you played the game I don't have to tell you how awesome that would be and how good fucking job would they do.

Yeah, I love them too, but them being bad motherfuckers is the very reason I love them.
Image a land quite far up north, damp because of it's low altitudes and covered mostly with dark pine woods, murky lakes and bogs, with only small strips of cleared land between.
Imagine a network of gaunt, tall gothic fortresses of red brick, build upon those land as a system of military occupation.
Imagine an hierarchical society with horned-helmeted, uniformly clad knights at the top. Those knights are gods and masters for everyone else, yet even among their ranks there is strict hierarchy, and disobedience is punished strictly.
Below them are knights that aren't fully part of the order, or aren't at all, instead being their secular vassals. They may poses land and wealth but they have very little to say in politics, as the Order controlls the state like a mono-party proto-fascist oligarchy.
Imagine burghers in very few scattered, mostly placed on cosasts of foggy Baltic, towns that don't have a faction of your usual burgher autonomy most cities in europe had at the time, also subjugated strongly by the Order, despite fact of their inhabitants being mostly settled ethnic Germans, often after original population (and that including christian Slavs) exterminated.
Imagine caste of Baltic serfs, being slaves in almost everything but their name. Most of them, or their ancestors, were forced to chose cross at swordpoint which differs harshly from many other crusades where knights just took the land and actual conversions were though offering boons and normal proselytizing. Slightest disobedience towards the Order is punished by death. Revolts are bathed in blood and through the exceptional organization of the order and their fortress network, none of them stands slightest chance.
Teutonic Order State is literal paragon of LE alignment, and Teutonic Knights are your prototype evil knights. It wouldn't be out of place as order dedicated to Bane or Hextor

Well, my understanding is that they were not necessarily actually lepers, but rather an order founded around protecting and caring for a leper hospital.
Still, the famous Baldwin IV of Jerusalem suffered from leprosy and was a famed military commander who participated on the front lines before his illness crippled him, so it's not unbelievable that there might have been some knights who suffered from that terrible condition too.
