Well, most of the Imperium's problems are reactive to external forces like xenos and Chaos that keep trying fuck with the wellbeing of humanity. All the totalitarian bureaucratic bullshit is a result of both the Imperium's gargantuan scale and the numberless threats and obstacles plaguing it. It's reasonable to assume that things would run slightly smoother and with less grimdark if all the aliens and daemons around it weren't trying to kill and eat every human in the galaxy all the time. And yet the Imperium still gets things done at a plot-relevant pace. Congress gets less shit done despite the United States being one of the coziest countries to live in. Almost all of its problems are internally corruptive.
