The first twenty is a crit. The second 20 (for the confirm) is a kill. A third 20 (to confirm the second) means you successfully completed the story arc. The fourth 20 (to confirm the third) means you finishes the Campaign.

I'd just make it spectacular, maybe give the player some small mechanical bonuses to show for it (e.g. +1d4 to all unarmed damage or something); you know, something small to show he achieved something, yet not big enough to make the other players complain, though your group may vary, of course. Honestly, I always houseruled out the 'roll to confirm 20' thing myself.

Trip20 is usually considered a killing blow by most house rules. Nat20 is a guaranteed hit and a critical threat. Confirmation roll Nat 20 is usually max damage on the crit and house rules prompt a third roll. Third Nat20 in a row the kill. Beyond that is your own nonsense.

Two 20's during a fight cause something the player possesses to gain an enchantment, usually related to whatever the PC was fighting. I guess they'd be pretty decked out if they managed to roll six in one fight, let alone in a row. Might just give the PC themselves some fancy magical ability at that point.

Besides asking nicely, I'd just say let it die down naturally.

As for what to do with that many 20's? Just make it cool. Assuming D&D fighter (Urgh) power levels, have the orc's body just evaporate and coat the walls. I did that when my player's 1st level fighter (with 20str) crit and rolled really high with his greatsword against a mook with 8HP. Other than that, I'd say don't have it come up in-universe anymore. Let the players joke around and just have people assume any rumors of such are exaggerated or made up. If you've got a decent group, it'll stop.
