A mysterious giant sea-life washes up on the shore of a small fishing village. From hearsay apparently the people have taken to worshiping it as some kind of god. While it is very weak it's apparently still alive.

How does your party react to and approach this?

Well, that's entire up to you. The reason so many Old Ones were able to be taken advantage of is because they had undeveloped minds or were made weak. Rom was literally without thought or understanding, blocking the Moon Presence and Amygdala and all that goofery from spilling into the waking world. Mergo was an infant, something the School of Mensis wanted to exploit to gain higher understanding. Ebrietas was 'left behind' by her fellow Great Ones and was similarly exploited by the Choir and the Healing Church. Mother Kos was found on a beach, pregnant, and had all her followers hunted down, experimented on, killed, and her child was stolen from her. In fact, the Kos Parasite looks a lot like an infant Great One itself. For all we know, the phantasms in her body might be more of her children, stolen and experimented on.

The difference with you is that you aren't stunted or weak. You know the follies and crimes of those around you, you've witnessed all the deception and lies and horror in this mad grasp for power made by Byrgenwerth and its spawn and even more than that, you've grown to rival the Great Ones in insight by consuming the umbilical cords. You could be the greatest of the Great Ones, achieving true enlightenment and a safe haven to call your own and none left to seek you. The choice of what comes next is entirely yours, good hunter.

A careful balancing act between being able to see the bigger picture while being slightly apart from it ensues. One theory I heard is that the entire town of Yahar'Gul was sacrificed in an attempt to bring back an Old One's dead child, and the result was the one reborn.

The theory was that the town was flooded with a special blood that was meant to convert the populace into a new body for the child, but instead transported everyone it touched to the exact same spot and telefragged each other into a horrible franken undead thing.

The reasoning for this are the statues clinging to the walls can be seen as people fleeing something in the street as well as the others which seem to be praying or cowering, which signify fear at impending death. Another part of this reasoning is that pool of blood that works a a teleporter.

Gehrman also sleeps better when you kill the Orphan, because he's no longer trapped in the Fishing Village dream. He's punished for his sin of killing Kos, who was alive when she washed up on the shore and killed when the Hunters raided (according to the strategy guide).

They also fight similarly, using a curved weapon in a fast and highly aggressive manner involving lots of leaps and bounds.

Orphan is what Gerhman would "really" look like if we had eyes to see, a surrogate infant to an inhuman beast more Kin than man.

The Paleblood Hunt doc speculates that Ebrietas may be an ascended human like Rom was, and left behind (or rather ignored) because she was not a proper Great One.
I like the sound of that, but there's not a lot evidence leaning either way.

Assuming, of course, that the Moon Presence has to maintain her existence. A machine doesn't fall apart just because the guy who built it dies, after all. And even if she is wholly dependent on the power of a Great One to maintain her existence, who's to say the newborn Great One (the player character) didn't just take over that role?

Maybe the Orphan is to Kos and the Hunter's Nightmare what Gehrman is to the Moon Presence and the Hunter's Dream. And who knows, maybe Kos actually based it on Gehrman.

Or perhaps the Orphan is what you get from a human father and a Great One mother, as a foil to Mergo, with the Great One father and the human mother.

Or hell, both.

The Orphan of Kos seems to be an inverted hunter; made to hunt hunters as is the theme of the curse that is the Hunters Nightmare.
That it was patterned off of Gherman, the First hunter, only makes sense.

The orphan actually fills the role of the moon presence.

Each dream/nightmare needs a Host, and a Source. Gehrman was the host, moon presence was the source. Micolash was the host, Mergo was the source. Maria was "probably" the hose (could also be the buttpained guy at the beginning of the hamlet), Orphan was the source.

People forget that Orphan was a great one just like Kos was. He's actually a very important character in bloodborne since he cursed the shit out of the church to avenge all the fishmen the church was poaching, thus leading to infected blood and the original plot of bloodborne.

I suppose it depends on the person, or ow much blood you're getting, or how involved you are ministration. Usually, I think its a protracted transformation, but a very physical one.

The regular Yharnamites you fight are all fucked up, but not too much. They're like exaggerated Lon Chaney Wolfmen. Notice one arm is actually longer than the other, so it isn't an even transformation, either. They're the first stage because they're still pretty coherent.

Then those troll guys, I think that's either down to particular people, naturally big brutish guys, or it's the third stage. You go from Lon Chaney, to tall wolfey guys (like you fight near the sewers in Yharnam) to trolls and then full on Werewolves. Then particularly strong people become Cleric Beasts and Blood-Starveds, etc.

But then there's the actual church ministers, like Amelia. I'm guessing we never got enough of a proper look at her. Gascoigne, pre-beast mode, looked human enough, he was just big. I'm guessing Amelia probably had some beastliness to her we didn't get a look at. Maybe church ministers and/or Hunters can repress the outward physical symptoms of the Scourge, but eventually, it hits full force.

Miyazaki has said that the potency of the curse's transformation has a lot to do with human virtue and that the whiplash from human to monster is dependent on how much you cling to humanity. Amelia he cites as an example of someone who clung on tightly for a long time and then WOOMPH suddenly giant fox-mummy-thing.

In contrast, I see people like the troll guys and Gascoigne, who stay basically human but taken on a more savage and brutish appearance, are people who put up basically no real fight and accepted early on the curse for the increased physical power it provided.

the Healing church is only located in Yharnam. that is the whole reason that the PC goes there in the first place, because Blood Healing only happens in Yharnam. the scourge is only so prevalent there because of blood ministration, a massive amount of beasts compared to the rest of the world because it stems form blood healing

One theory is that the different beast forms stem from different blood sources, but it only goes as far as beastial(Healing Church), alien(Choir), and insectoid(Vilebloods). I really want to DM a campaign inspired by Bloodborne and other eldritch horror very badly sometime. I was thinking the catalysts be magic rings that all give powers to whoever wears them, but when you put them on and continue wearing them, the powers slowly grow out of control and give birth to nightmarish planar creatures of unknown origin. I was thinking of a few ideas for rings, like a ring that makes the wearer invisible, but the more the person wears it, the more their shadow begins to do things that they don't do, and it begins to assimilate and devour other shadows of objects and people. After it gathers enough, the next time the wearer puts the ring on, his shadow disappears and is no longer there even when the ring is off.

I want the implication to be that it has appeared somewhere in our world and has taken on the attributes of the shadows of things it has devoured and now it hunts with un-satiated hunger.

Honestly I doubt the healing blood finds its way out of Yharnam very often. The Church is both secretive and militant about ministration, and setting up another paleblood garden far from the source is doubtably very feasable. But there has to be at least some bleed through, as Valtr and his constables chased a beast INTO Yharnam, not out of it.
I'd conjecture that outside of Yharnam, the beast scourge is just a persistant rumor until some accident happens.

Dream Hunters can't change, they are immune to the affect of the scourge

remember at the beginging of the game when you first take the blood healing and the werewolf pops out of the pool of blood? when it touches you it bursts into flame because you are immune to the disease because of something in your blood. right after that happens, the messengers crawl over your body then you pass out, signaling your transformation into a Hunter of the Dream.

Durja was a Dream Hunter and he never went crazy with the beast blood despite being in ground zero in Old Yahrnam even after he stopped dreaming. Ellien never went blood crazy either because she was also a dream hunter at one point.

That is somewhat true.

Beasts exist because Orphan cursed the healing church and the blood became unreliable.

The aliens were given great one blood, probably from Kos, that turned them into the plip plops originally, and eventually the "perfected" celestial children.

The insectoids in byrgenwerth are just a result of trying to add more eyes to see beyond, Willem's hypothesis. No idea if they did that by using vileblood blood.

That's fair enough. But Cainhurst is far enough from Yharnam apparently the moon doesn't affect it, yet it's still filled with monsters. Couldn't blood have stolen out of Yharnam via conspiracy to some other land, and it naturally goes wrong once there because they don't really know what they're doing.

I dunno, I just feel like there is more that could be done. It's a great setting.

If you want it more Bloodborne-y, make it illithid tpe grafts or symbiotes, something biological that's harder to root out or simply not use again. After all, one of the points is that Healing Blood is ubiquitous, used even for intoxication rather than illness or wounds.
Not to mention that the setting plays a huge role.

It is a good setting but Bloodborne and the DLC has told the whole fragmented story. I'm just not sure if theres another video game in there except for a prequel.

Other stories can be told in that universe through other medium sure, but as far as Bloodborne 2 I just don't know what they could do.

Another ring might be a ring of gluttony that lets the wearer eat huge amounts of food without getting full or gaining weight. At first it's okay to eat a ton, but after continued use, the wearer begins to eat more and more to become satisfied. At its final stage, the wearer eats so much that their stomach becomes grossly huge and they begin to feel its contents shifting around inside until a giant wormlike creature forces its way out through their belly button and scurries away. The wearer is unhurt, but a terrible thing is on the loose now.

The creature kills whatever animal it finds and tries to wear and slightly re-animate their corpse. Eventually it begins to kill humans.

This one kind of sounds a little fetishy, but I'm going more for body horror.

I kind of wanted it to be a "normal" seeming hook since the players would be used to the typical adventuring affairs and I think it would give them a diminished sense of security as it transpires. It's just a tiny idea though, and anything could introduce these things, really, but I like the idea of the horrible unknowable monsters using the players directly so it feels more personal.

If the players attempted to take off the ring I'd definitely make them do a will save of some kind.

I feel like the ring of invisibility doesn't actually make you invisible, it just sticks you halfway through the veil. Visually, you exist in this eldritch hellscape, physically, you exist here. But the more you wear it, the more you're sundering the veil and eventually it'll tear a hole open and the things that have been able to see but not get you can now get you, because every time you wear that ring, they swarm around you.
