2. That rule only applies to league games, in direct elimination you get eliminated if you do that (and you need 5 red cards, which is quite hard to get if not doing it on purpose)

not sure about red cards, but if psg had used an ineligible player, for example putting the physio on, they would have lost 3-0 instead of 6-1, and would have won on aggregate. but barca would probably have won the appeal.

I dont recall it ever happening in a "regular" game. It usually happens under suspicious matches (rigged) or when a really violent everyone against everyone fight happens.
I remember when you could get red cards in FIFA by tackling the goalkeeper while he was holding the ball. But I always made sure to have enough players to not lose automatically. Fun times.
