Well one cause of pain as a result of illness is the fact that many viruses utilize what's known as the lytic cycle in which viruses interacting with receptor proteins on cells enter into human cells and utilize its mechanisms to reproduce it's respective nucleic code whether it be DNA or RNA, single or double stranded. When cells are engaged in the lytic cycle they reproduce to the point where the cells fill and burst in effect releasing more viruses to your body. Over time this will happen to several different cells and therefore cause tissue damage thus making you feel as you say like a gaping asshole. Some viruses use what's known as the lysogenic cycle where they are incorporated into the DNA of individuals and are thus reproduced in that fashion. These are viruses such as Cold sores which come up every once in a while, Shingles as a result of an early childhood Chicken Pox, and HIV. It's not really applicable to what you're discussing at least not in the short term, but they can be triggered to enter into the lytic cycle and start destroying tissue as well as a result of certain stimulants depending on the cell. To fight these viruses, you're body first has a primary line of defense, basically you're skin and mucous membranes such as those in your lungs/throat and intestines serve as a way to sweep viruses and bacteria out of your system before you can be infected. When this fails your body resorts to a general response. This is the inflammatory response in which mast cells and basophils (Specific leukocytes) release histamines which dilate precapillary blood vessels and contract postcapillary blood vessels thus increasing blood flow to the infected area. This increase in blood flow moves other white blood cells such as neutrophils to hopefully remove the invader while similarly inducing the complement system which relies on a system of proteins to destroy the invaders before they can reproduce. are activated which trigger your body to increase it's temperature by a few degrees to hopefully slow the reproduction of the viruses to the point that generic macrophage/neutrophil attacks will be suffice. If this is not enough while this is occurring, another segment of the immune system is creating specific immune responses against the pathogen but that's not important here. Essentially the destruction of cells caused by invading cells and the inflammatory response and subsequent movement of fluids and pyrogens cause you to feel like shit but ultimately are what cure you of the disease and furthermore make you stronger as a whole in terms of your immune response. This was a very brief overview of course I could go into more detail but it's not needed for your question.
