I've reached a point where I just spend entire days looking at inane shit on the internet and smoking weed. I have this constant sense that I should know how to better spend my time but I have no clue how to go about changing my routine. It's driving me insane, I feel like I'm losing the ability to think clearly. What are more fulfilling ways to spend time as a NEET?

Work out.
Develop a hobby. Model building, cooking, biking, hiking.
Get outside as much as you can.

If you like drugs you might discover the joys of sadism. It's all about pleasure, and pain. Pain is the true nature of existence. Enjoy yourself, but make sure you will always share the pain. I've got the same problem. I'm about to start writing my dreams and surreal thoughts in a journal. Doesn't hurt to do so and it's a small step towards something. Doesn't really get in the way of my completely empty routine either. It's quite a conundrum, OP.
