Because its work, a good deal of work to stay clean and presentable. Dressing properly is another almost-trial. You need fatfuck clothing or everything is just going to cling and bulge out around you. It's not cheep either, fatty clothing is expensive and if you're low on money AND gorging yourself then you'll tend to wear what you have to ruin before replacing it.

Lastly fat is insidious, you become more disgusting in subtle stages that you adjust to without much thought. I used to be addicted to speed and vicodin, but when I stopped using them I became addicted to food. It most certainly is the same as being addicted to substances. An addictive personality is an addictive personality regardless of the drug of choice. Besides, I'm not sure why you think that being an alcoholic is worse than being fat.
