As for mcdonalds, what I did to get one fired before was I went to the website to file a complaint. They asked to meet me in person though, and when they did I reported that person's higher up lul saying he's retarded to think I would feel safe showing up face to face and then the manager got in trouble lmao

If the restaurant gives customers access to the machine, the cultural assumption is free refills.

If they don't want free refills, they keep it behind the counter and have employees fill cups.
