If it's localized like that you're obviously doing something weird. Either your shoes are too small or too big which creates friction, or you have a weird OCD move where you bully your toe so much it ends up like this. In any case it has to be something you're doing.

You should apply a moisturizer everyday.

I believe you're really fat, and the small amount of walking that you do causes enough friction so that a callus inevitably forms where you allocate your weight. Oddly you seem to support yourself on your toe
