Is a million people suffering mild annoyance worse than one person experiencing immense pain? Which should be sought to be avoided?
Can you stack the mild pain of the majority, or is the mere existence of that single, immense pain worse?
It all could be avoided if people just stopped reproducing.

As someone with cluster headaches.
The pain is so bad my eyes water and my face swells, I can't see out of my eye on the right sometimes.
I've had people tell me its the worst pain imaginable and I believe them.
I've had times though that i've been able to fight through it to drive something to town for my family's work.
I would say that knowing I can somewhat bear pain, I would do it for someone else who couldn't. I would only hope they don't take it for granted.

Assuming 1 million and 1 average people who aren't especially good or bad then I'd say the immense pain would be worse.
Immense pain will ruin that person's life while most people will be able to become habituated to the annoyance, like people do with loud noises or how you don't even notice when you get a small scratch.
