It's a mixture of affectation and hormonal problems. But mostly affectation.

To my knowledge it only occurs in English-speaking countries. I think it goes along with a more general trend among all gays, no matter the language, to want to appear feminine and young.
There's a documentary about it called "Do I sound gay" and it tries to show that it's fine to have a lisp or whathever, but in the same documentary a childhood friend of the main dude mentions that he didn't have a lisp until he got a group of gay friends.
On the other side I knew kids that spoke like fags since primary school so I guess that like most normie behavior it's just unconcious imitation.

One of the guys I went to school with came out as gay our junior year. He didn't have a gay accent as a little kid, and he didn't have a gay accent as a sophomore. But when we all came back junior year he was flaming gay and had a lisp.
