try meditation, perhaps you have an overactive mind that's preventing you from letting yourself rest completely, also i'd recommend trying herbal supplements and teas that assist in alleviating insomnia. try out stuff like lemon balm, valerian, kava kava, passion flower, etc

No
Yes
Marijuana once or twice a year, alcohol once a month or so
your body clock could be producing too much melatonin, could you not be drinking enough water? try a glass before bed

Tell me about the temperature sensitivity.
It's called depression ya fanook

Depression doesn't directly lead to chronic fatigue. Depression makes it hard for you to fall and stay asleep. OP didn't complain of either of those things.

With the B vitamin thing: Most of the time if someone is B deficient it's because of an absorption problem. In other words they don't digest the vitamin B and they just end up shitting out the contents of the pills.
You can get sublingual (under the tongue) vitamin B that gets absorbed directly. It still might not solve your issue, but it's worth a try as they are cheap online or at any drugstore. It also usually tastes good.
