Careful about small talk, it's ok to break up a really awkward situation but you need to be able to then go on with more interesting topics.
Ex: oh the weather's nice isn't it?
It's the perfect weather to play football/Read a book in a park... do you like (blank)?
I'm shit at dating but it worked for me once.

Well there is small talk, it is often such a brain dead topic that it allows you to try and think of something more interesting go talk about. Taking advantage of the years apart might be good.
"So its been so many years, what have you been doing?" "Where are you working?" "Do you like it there?" Basic shit.
As for the deeper conversation I cannot quite help because that will either happen or it won't and that isn't because of you or her its just sometimes things click together nicely or they won't.
If you have the money, time and confidence you could always ask if she'd like to go for something to eat. That will naturally calm you down as you will be eating and eating settles the nerves. It also gives you more to discuss I.e the place you're eating in, the food, the service, shit like that.
