Get started on your biggest issues, best thing to work on for beginners are traumatic memories. You'll see the quickest success with those.
From what you've listed I understand that you want to implement behavioral changes. Take your time and be as specific as possible.
You'll eventually notice your attitude changing.

I was in a very dark place not so long ago. Had no self esteem and no motivation to do anything, crying to myself in my desk when sudden realization of the situation used to come. I couldn't go to the barber to get a haricut because my self loathing was so big I couldn't stand looking at myself on the mirror for longer than two minutes.

Failing school, losing weight because I hated having to get out of the house to buy food, close to zero social activities because of the same reason.

It has gotten a little bit better I think. I still have my insecurities but now I can stand going to the barber to get a haircut, which is something I'm really proud of myself. I'm starting to eat better and getting into a healthy BMI.

I don't know why I wrote this, just wanted to share I guess. It's all about baby steps, baby steps. It doesn't matter how small it is (like getting a haircut or going to the supermarket) everything counts. Treat it like any other illness that needs daily care and treatment until it gets better, that has been my experience until now at least. I hope one day I can get better.
